Methamphetamine (MA) abuse is a significant public health issue in Malaysia, mainly due to its severe cardiovascular complications. Intravenous (IV) use is associated with heightened health risks, including accelerated development of cardiomyopathy and other cardiovascular diseases. We present a case of a 35-year-old man with a history of chronic MA abuse who experienced typical angina, heart failure, and aggressive behaviour. He had recently switched from an oral to a self-prepared intravenous MA route. He was diagnosed with methamphetamine-induced cardiomyopathy (MaCM). This case illustrates the escalation of CCF development and psychiatric consequences associated with the transition from oral to IV MA use. Early intervention and comprehensive, multidisciplinary management are crucial in addressing the increased risks posed by IV administration, particularly in preventing the rapid progression of severe cardiovascular complications.
